Information for Referrers

Thank you for considering Melbourne Health Group for your patients and entrusting us with their care. We will manage their care and ensure that your recommendation and trust is well placed. We will dedicate our time to ensure that your patients are provided with the highest quality physio care to ensure they’re on the best path forward for them.

GP/EPC Referrals

Please email or fax your referrals to the details in our contact page.

Patients with a valid concession card will be BULK BILLED for their plans.

All other clients will have an out of pocket of $15 per session.

WorkSafe, TAC, DVA

Please forward all appropriate records via email or fax to ensure the best possible care is provided to your patient.
